    In Right at Home, we are looking for a compassionate Social Worker/Care Coordinator to help serve the communities that we have presence in by assisting our clients and caregivers when is needed.

    You will be the responsible to guide our clients to a better-quality life with the help of the caregivers, for this you will oversee assessing clients and gathering relevant information about their specific case, and work as a team with their families, community members and the Right at Home team.

    Social Worker/Care Coordinator Responsibilities:

    Create clients experience, through communication with clients, families, care partners and referral sources.

    Plays an integral role in interfacing with clients and families to ensure safe, appropriate, and meaningful care is being provided.

    Orients new caregivers and serves as their mentor while supporting their care delivery.

    Maintains a calm, positive demeanor always and can effectively multitask in a high functioning office environment.

    Answers phone calls from clients, caregivers, and new prospective clients with the goal of converting to actual clients.

    The desired social worker/care coordinator candidate will possess the following:

    Have a valid driver’s license and insured automobile.

    Bachelor's degree in psychology, social work, or related field (preferred).

    Work experience working in social work and/or in the home care industry or medical office setting.

    Knowledge and ability to develop treatment plan, understanding the needs of the client.

    Comfortable using a computer for various tasks, including Word, Excel, Outlook, and Power Point.

    Be available as required for on-call duty outside of normal office hours.
