JOIN THE MOVEMENT
First and foremost, we’ve got to find other changemakers who are ready to join our movement. We’re looking for Nurse Practitioners (NP), Physician Assistants (PAs), Licensed Clinical Social Workers (LCSW), and Care Coordinators (CNAs, MAs, CHWs, care managers) who believe serving seniors IS their calling, their reason for jumping out of bed every day. Our ambition is to create the nirvana workplace for this tribe of specialty clinicians and give them every imaginable tool and support to work at the top of their license. And, most importantly, help them make a giant difference in the lives of their patients. Because “if not us, then who?”
THE ROLE
Working in small teams (Care Pods) with other highly capable and passionate team members you will:
● Be a core part of the care team, guiding patients and their families on the journey of dementia and late life mental illness
● Participate in chronic care management of patients, families and caregivers
● Assist in determining type and duration of services necessary to meet client needs
● Complete screening assessments over the phone
● Facilitate the provision of in-home, tele-presentation/virtual, and telephone support services to clients
● Assist with care coordination of patient care with primary care, specialists and home health aides
● Coordinates patient care with Rippl social workers and clinicians (NPs, LCSWs, Medical Director)
● Assist in the organization of family resources for the effective care of the client
● Follow up with patients and caregivers to monitor treatment progress
● Routinely interact with other personnel as well as clients, family members, caregivers, community providers/referral sources, case managers and other third-party resources.
● Performs other administrative duties as assigned
WHAT YOU’LL BRING
● Passion for working with older adults and their families
● High School diploma or GED required
● Completion of CHW training program (preferred but not required)
● 1-2 years previous healthcare experience (CNA, MA, CHW, Care Manager) -or- customer service required
● Strong computer literacy, including proficiency in Microsoft Word, Excel, and PowerPoint. Will need to master additional software including our EHR platform
● Demonstrate ability to use assessment tools in order to identify patient or individual needs
● Excellent interpersonal skills
● Ability to maintain a high level of confidentiality
● Excellent organizational skills
● Demonstrate the ability to develop a resource manual of internal and community-based resources and referral opportunities
● Must possess above average human relations, customer service, problem solving, and administrative skills
● Strong relationship building skills with people from diverse and varied backgrounds and education levels.
