    PIH Health is a nonprofit, regional healthcare network that serves approximately 3 million residents in Los Angeles County, Orange County, and San Gabriel Valley region.     The Clinical Coordinator shall be responsible for mentoring, supervising, and evaluating the clinical competence of the pharmacy staff. He/she shall integrate clinical skills and assess the involvement of the pharmacy staff to meet the requirements of core measures and medication management standards. He/she shall work in a collaborative manner to develop evidence-based clinical pharmacy medication dosing guidelines for implementation upon approval from the appropriate medical staff committee(s). He/she shall be responsible for developing the unit based pharmacist concept, which includes rounding, collaboration with the multidisciplinary team, clinical interventions, and medication safety across the continuum. He/she shall oversee, develop, educate to, and assess compliance with, the clinical pharmacy documentation system. He/she shall continually develop the pharmacy technology-driven systems to improve patient safety and streamline workflow. He/she shall compile reports and complete projects adhering to deadlines set forth. He/she shall be responsible for quality reports, medication utilization evaluations, ensure pharmacy representation in the development of medication use management programs including clinical guideline development, critical pathways, disease management, and medication use programs. He/she shall be responsible for the PGY-1 Pharmacy Practice Residency program and pharmacy student rotations, including training and supporting residency/student preceptors, and participating as a preceptor when needed. He/she shall disseminate pertinent information to the pharmacy department, nursing, physicians, and other clinical staff deemed appropriate on side effects and proper use of all new medications added to the formulary, medication shortages/substitutions, and formulary deletions. He/she shall serve as a liaison between medical staff, patient care areas, and the pharmacy staff, and will resolve situations that may compromise the integrity and professional reputation of PIH Health and Pharmacy Department immediately.  

    Required Skills

    • Advanced knowledge of pharmacokinetics, and clinical knowledge as it pertains to oncology and the ability to apply them in the hospital setting.
    •  Critical thinking skills.
    • Knowledge of policies governing the use of medications, protocols, evidence of continuous professional training in a related field of work, excellent computer, verbal/written communications, and the ability to prioritize is required.
    • With consideration to age, utilizes the approved process to resolve the biophysical, psychological, educational, and environmental needs of the patient/significant other when administering care.

    Required Experience

    • Graduate of an accredited school of pharmacy recognized by the California State Board of Pharmacy.
    • Must have a Pharm.D. degree.
    • Must be a California Registered Pharmacist. 
    • Completion of a hospital residency and/or advanced certification in a pharmacy specialty area is preferred.
    • Must have a current BCLS Healthcare Provider CPR Card from the American Heart Association.
    • Must have at least three years of hospital pharmacy experience.
    • Experience with the design of pharmacy systems and processes is preferred.
