    Patient Care Coordinator 

    JOB SUMMARY: Provides patient care within the prevailing state standards of practice and according to the policies of the facility. Communicates appropriate information to other team members. Shall be knowledgeable of HIPPA regulations concerning protected healthcare information and other federal guidelines regarding confidentiality. Shall be flexible to perform a variety of duties. This position requires strong interpersonal skills to regularly interact professionally, promptly and courteously with patients, physicians, and their office staff in addition to other Center personnel. Helps maintain equipment, supplies, and cleanliness of facilities as a whole. M ay have additional responsibilities as designated by the Team Lead or Clinical Director, which include but are not limited to, serving on committees, participating in staff meetings, projects, and QAPI.

    JOB TITLE: Perioperative Nursing Assistant

    1. ESSENTIAL JOB DUTIES AND RESPONSIBILITIES: The following description of the job responsibilities and performance expectations are intended to reflect the major responsibilities of the job, but is not intended to describe the minor duties or other responsibilities as may be assigned from time to time.

    A. Knows and adheres to policies of the Center and demonstrates fiscal responsibility.

    B. Follows standards, policies, procedures, goals, and objectives for perioperative nursing care according to accepted standards.

    C. Maintains and demonstrates current skills and knowledge in aseptic technique.

    D. Demonstrates ability to understand the unique needs of each age group of the patient population.

    E. Observes proper body mechanics while performing work area duties and patient care, including transporting.

    F. Provides direct patient care by assisting patients with dressing and undress, changing linens, getting in and out of bed, ambulation, etc. Assists with patient discharge by transporting patients to their vehicles.

    G. Collect and transports specimens, including urine, blood, and gastrointestinal, etc.

    H. Obtains and documents patient’s vital signs, height and weight, and reports results and observations.

    I. Remains calm and functions as a team member during times of stress and crisis situations.

    J. Answers and responds to patient call lights and requests.

    K. Escorts visitors to designated person or areas.

    L. Assists in maintaining patient charts.

    M. Performs center and patient support duties including running errands, picking up supplies, delivering specimens, unpacking and storing supplies, counting inventory, and assisting with data collection on various clinical logs.

    N. Assists with monitoring and maintaining adequate inventory of supplies in Pre/Post areas.

    O. Assists with monitoring of equipment repairs and maintenance and maintains binder with documentation of repair requests and completion of jobs.

    P. Complies with Policies and Programs, adheres to safety guidelines, and ensures a safe work and patient care environment.

    Q. Maintains patient, physician, and employee privacy and confidentiality per policy and HIPPA regulations.

    R. Informs Team Lead of applicable day-to-day situations.

    S. Shall be flexible, reliable, productive, patient-oriented and self-motivated.

    T. Ability to work as a team member.

    U. Motivates employees to maximize their performance. Encourages a work atmosphere that promotes efficiency, productivity, accountability, and excellence.

    1. MAINTAINS A SAFE ENVIRONMENT FOR PATIENTS AND PERSONNEL.

    A. Ensures continued compliance with regulatory agencies, i.e. CMS, OSHA, CDC, TJC.

    B. Implements safe practices and evaluation of the environment of care management including life safety, security, hazardous materials and waste management, emergency equipment and utilities.

     Assists with environmental hazard and infection control surveillance, and participates in emergency preparedness drills.

    1. Assists in the provision and control of supplies and equipment needed for optimal operation of the Facility.

    A. Assists in the control inventory, drugs, and supplies.

    B. Observes and communicates needs for new and different items.

    C. Uses and handles supplies to decrease waste, mishandling, or incorrect methods.

    D. Assists with quarterly inventory.

    1. Assists Business Office Manager with administrative duties to ensure proper functioning of all clinical care departments.

    A. Follows the Center’s professional code of conduct and dress code policy.

    B. Communicates effectively and courteously with visitors, physicians, their office staff, patients, patient’s family and employees.

    C. Attends mandatory in-services and meetings.

    QUALIFICATIONS: To perform this job successfully, an individual shall be able to perform each essential duty satisfactorily.     PRESCHOOL LEAD TEACHER REQUIRED SKILLS:

    • High school diploma or GED required
    • Childhood Development Associate (CDA) or degree in Early Childhood Education (ECE) preferred
    • Must meet Illinois-state school-age or preschool teacher licensing requirements
    • Previous hands-on preschool teaching or classroom leadership experience preferred
    • High level of flexibility and willingness to work all shifts
    • Must be at least 18 years old
