    Job Title: PRP Worker

    Program: Psychiatric Rehabilitation Program

    Exempt/Non-Exempt: Non-Exempt

    Compensation: Fee-For-Service

    Anticipated Travel: 25% of the time. There is no additional compensation for travel.

    The PRP Worker is responsible for the direct provision of rehabilitation and support services to participating consumers. PRP Workers visit consumers in their homes and communities, and provide services in the home, community, or program site.

    As The PRP Worker, You Will:

    Conduct regularly scheduled consumer visits.

    Provide identified PRP rehabilitation and support services in accordance with the consumer's Individual Rehabilitation Plan (IRP).

    Conduct Rehabilitation groups and activities to address rehabilitation and support service needs.

    Maintain required and consistent documentation for each client, including contact/progress notes, monthly summaries, and quarterly reviews.

    Participate in ongoing trainings and supervisions.

    Maintaining a client caseload of a minimum of 20 clients

    Work from a Client-Centered Approach

    Understand the importance of self-determination in meeting a client’s needs

    A minimum of 2-years direct care experience with adults and youth

    Determine if the client has, or maybe eligible, for Federal or State entitlements

    Assist the client, if necessary, to apply for all entitlements for which the client may be eligible if they do not have entitlements.

    Encourage community integration and access to local support services

    Goal setting and treatment plan development

    Asses the client’s strengths, needs, abilities, and progress

    Provide supportive services that encourage independence

    Medication monitoring and crisis intervention

    Willing to submit to random drug testing and criminal background checks

    The PRP Worker Requires:

    HS Diploma (or GED) REQUIRED - Associate or Bachelor’s degree preferred

    Minimum of two years direct care work with adults and youth preferred

    Strong writing skills

    Ability to work independently and manage a productive schedule

    Thorough understanding of HIPPA

    Valid driver’s license and reliable vehicle

    CPR certification
