  • Responsibilities

    The Clinical Care Coordinator is responsible for conducting Clinical Care Services in accordance with VOA Chesapeake’s operation as a Level I Substance Use Provider. In this role, the Clinical Care Coordinator provides client services that are essential for assisting the client in the achievement of rehabilitation and recovery goals as defined in the client’s Individualized Service Plan. The Clinical Care Coordinator has the overall responsibility for the development and implementation of the client’s treatment plan.

     

    REQUIREMENTS:

    Required: Candidates must have a Bachelor’s degree and have at least two years of experience in a relevant human service field. A minimum requirement of at least one-year of related human service activities. Excellent writing skills, knowledge of computer, must have own transportation. Candidate must be a licensed or certified qualified practitioner (QP) MUST BE A PSYCHOLOGISTS, LICSW, LGSW, APRN, RN, LISWS, LPC, OR LMFT

     

    Preferred: Candidates possess Master’s Degree in the health or human service field. Must be a licensed or certified qualified practitioner (QP) Candidates must have at least two years’ experience providing services as a licensed clinician or at minimum one year as a resident under supervision of a licensed professional counselor, licensed social worker, or a certified addiction counselor II, specifically working with persons experiencing substance use disorders (SUD), serious mental health illness(SMI) or co-occurring disorders (COD); Experience with community based services and multi-cultural setting is preferred.

     

    Successful candidates must be able to develop effective working relationships with the client population, be self-motivated and able to motivate others, be able to relate to individuals making significant life decision, possess strong organizational skills, and have strong oral and written communication skills. The individual must also be computer proficient.

    OTHER:

    • This position requires driving the company vehicle (up to 15 passengers) and/or your personal vehicle for company business including transporting clients/residents. Must be privileged to drive according to the criteria set forth by Agency MVR guidelines.

    • Valid driver’s license in jurisdiction of residence

    • Vehicle liability coverage if driving personal vehicle (will be required to transport clients in personal vehicle)

    • May be required to drive a van

    • Acceptable DOH background check, FBI Fingerprint, OIG Exclusion check

    • Negative Drug/TB Screening and Health Certificate

    • CPR/First Aid Certification must be acquired and maintained once employed

    • PRINCIPAL ACTIVITIES:

    • Manage a caseload of approximately 75 adults

    • The Clinical Care Coordinator is responsible for ensuring that the client is at the appropriate level of care.

    • Responsible for linking clients as they transition through the levels of care, ensuring that the treatment plan is formulated.

    • Provides oversight of linkages to off-site services to meet additional needs related to a co-occurring medical and/or psychiatric condition, as documented in the treatment plan.

    • Monitor the compliance with, and effectiveness of, services over the treatment period and make a determination of the frequency of ongoing assessments.

    • Provide Individual, Family, and Group Counseling, including Group Counseling-Psychoeducation, according to the client’s assessed needs.      

    • Provide Initial, Comprehensive, Ongoing, and Brief assessments

    • Provide Crisis intervention services

    • Conduct education to include training on HIV to assigned clients

    • Attend comprehensive treatment team and assessment meetings on each client as part of the interdisciplinary team

    • Schedule clients for ongoing assessments

    • Facilitates implementation of treatment plan and administrative facilitation of the client’s service needs, including but not limited to scheduling of appointments, assisting in completing applications, facilitating transportation, tracking appointments, and collecting information about the client’s progress.

    • Assist the client in the development of interpersonal and community coping/life skills necessary to remain sober.

    • Assisting with providing access to testing and referrals for HIV and infectious diseases and coordination of services with medical care or specialty services related to an infectious disease

    • Assist the client in the development of relapse and symptom management strategies and plans.

    • Transport or coordinate transportation for clients as needed

    • Adheres to Volunteers of America Chesapeake’s documentation requirements

    • Documents all billable activities IAW MHRS/VOAC Standards

    • Complete billing/Progress notes weekly

    • Establish and maintain a community resources and referral index.

    • Perform duties as outlined in Program’s Operation Manual.

    • Perform duties as assigned.

    • Excellent computer skills including an intermediate knowledge of MS Office
    • Experience managing budgets in excess of $25-50M a year
    • Working knowledge of Super circulars
    • Knowledge of relational databases preferred, i.e., SAP, PeopleSoft, Oracle, Fluxx, EasyGrants, Salesforce
