    STGi is currently seeking a qualified Case Manager candidates for the Department of Housing and Urban Development (HUD) and Veterans Affairs (VA) Supportive Housing (HUD/VASH) Program.

    The Registered Nurse Case Manager will provide case management and coordinate the appropriate medical and mental health services for Veterans in the program. The RN will also provide consultation to other HUD-VASH staff related to the medical treatment needs of Veterans. The RN will provide direct nursing care as needed.

    ESSENTIAL FUNCTIONS:

    Clinical Services and Supports:

    • Work directly with HUD/VASH Clinical Supervisor and HUD/VASH Treatment Team to provide comprehensive discipline-specific services to Veterans and their families. Responsibilities include the management, coordination, and provision of discipline specific service to Veterans and their families in inpatient, outpatient, and community settings.
    • Work with case managers to develop appropriate treatment plans which will focus on the Veteran’s health and wellness as well as maintenance of permanent, stable housing.
    • Provide case management to participants, as needed, throughout the continuum of care.
    • Provide a full range of discipline specific services within commonly accepted standards of professional practice which includes case work, discharge planning, case management, and other services as appropriate.
    • Address Advance Directives in accordance with the standards of assigned work.
    • Provide individualized and/or family education and/or support groups with a focus on self-management, prevention, and wellness to ensure the Veteran’s continued success.
    • As assigned, supervise the practice of nursing students, peer support staff, or newly hired case managers.
    • Conduct nursing assessments to determine medical status and needs as well as psychosocial conditions of participants and family members.
    • Participate in discharge planning with other treatment team members and maintain responsibility for discharge plans including timely and appropriate execution.
    • Provide Veteran psycho-educational groups as deemed necessary and appropriate.
    • Provide consultation/education to participants on community resources, advance directives, and VA benefits.
    • Act as a consultant to other team/staff members, including providing professional development as deemed appropriate.
    • Refer participants and family members to resources for coping and dealing with the loss and grief experiences in disability, terminal illness, and death.
    • Provide outreach services to engage homeless Veterans, especially those who are chronically homeless with complex medical and mental health needs.
    • Assist in the navigation of healthcare systems including providing referrals or advocacy to support the achievement of patient outcomes. This may include accompanying Veterans to appointments to facilitate information sharing.
    • Provide direct nursing care as needed, including administering injections and treatments prescribed by a licensed provider, develop therapeutic relationship with Veteran to help them explore his/her condition on overall health, functioning, relationships, and housing access/sustainment and advance strategies to accessed needed care.
    • Assist Veterans by simplifying their treatment recommendations and medication regimens to promote Veteran agreement and cooperation to enhance positive treat outcomes.

    Quality Assurance and Reporting:

    • Collect and submit participant data per contract requirements.
    • Perform accountability functions including documentation of all contacts with program participants and completion of statistical and data reports in accordance with contract requirements.
    • Maintain data and statistical complication to comply with Joint Commission, CARF, and other VA policies and procedures.
    • Maintain familiarity and responsibility for all performance measures related to the HUD/VASH program.
    • Document interventions and activities in participant’s clinical record.

    Required Skills

    • Demonstrated ability to perform under circumstances of possible emotional stress and conflict, including dealing with un-cooperative clients.
    • Demonstrated expertise and experience in clinical assessment, interpersonal, and intervention skills.
    • Demonstrated ability to work effectively with homeless and mentally ill population.
    • Knowledge of/or experience with DSM diagnoses, assessment of level of functioning, EBP’s such as Motivational Interviewing, Critical Time Intervention, DBT, Harm Reduction, Cognitive Behavioral Therapies, Trauma Informed Care, Housing First, PTSD, recovery model, etc.
    • Ability to work collaboratively with others and in a team.
    • Takes initiative and has a solution focused practice.
    • Uses good time management skills and resources to balance case load direct service and paperwork.
    • Excellent verbal and nonverbal communication skills.
    • Must have a current California driver’s license and insurance for regular driving and transporting.

    Required Experience

    • A BSN with approximately 2-3 years of nursing practice/experience; or ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; or a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience
    • Experience working with the homeless population for a minimum of three (3) years.
    • Experience with Continuum of Care and Coordinated Entry System preferred.

    WORKING CONDITIONS/PHYSICAL REQUIREMENTS:

    • Involves considerable amount of standing, walking, stooping, bending, reaching, lifting, and turning.
    • Requires carrying or lifting of objects weighing up to 50 lbs., but occasionally may require work that involves pushing or lifting in excess of 50 pounds and going up and down flights of stairs.
    • The position requires visual and auditory acuity at a level to provide safe and effective nursing care.
    • The position requires the nurse to work for periods of time with arms above shoulder level or with neck in a fully flexed or extended position and the ability to physically control or defend against emotionally ill clients.
    • Requires use of hands for actions such as typing, simple grasping, fine manipulation, and repetitive actions.
    • Ability to distinguish shades of colors.
    • Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds.
    • Possible exposure to inclement weather during visits with veterans in their community
    • Work with people experiencing homelessness whether on the streets, in shelters or other places of habitation or services, may present inherent challenges and difficulties such as: exposure to bed bugs or other infestations, unpleasant smells or odors, individuals who may have poor hygiene or unclean homes due to homelessness, mental health symptoms, or poverty.
    • Additionally, employees may encounter instances of profanity, sexually explicit or derogatory language, or verbal or physical expressions of anger and trauma. Very rarely do these situations ever escalate to physical contact, and negative or derogatory communication patterns can often be negotiated successfully. All direct care staff are provided adequate training to develop skills to deescalate crisis situations that may arise, and STGi has established procedural safeguards for all employees to mitigate (but not eliminate) these inherent risks of employment.
