Job Description for a Diabetes Infusion Center
ROLE
Responsible for Managing Chronic Care Disease Treatments
Assessment and Evaluation of Health Status
Diagnosis and Coordinating health care for the diabetic patient in accordance with State and Federal rules and regulations and the nursing standards of care
Development and Implementation of plan of care and treatment
ESSENTIAL RESPONSIBILITIES
Will report to the site’s local medical directors
Will provide Infusion Therapy oversight to RNs and LPNs on site and will assist if needed
Perform age-appropriate history and physical examination for complex acute and chronically ill diabetic patients
Chairside education should consist of but not limited to:
Orders and interprets diagnostic and therapeutic tests relative to patient’s age-specific needs
Prescribes appropriate pharmacologic and non-pharmacologic treatment modalities
Implements interventions to support the patient to regain or maintain physiologic stability
Document Notes in EMR to be overseen and signed off by the Medical Director
MINIMUM QUALIFICATIONS
Graduate of an accredited school of nursing
Graduate of an accredited Nurse Practitioner Program or Physician Assistant Program
Current Advanced Practice Registered Nurse or Physician Assistant Licensure
Current in Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS)
Strong computer skills, including Word, Excel, and PowerPoint
EXPERIENCE Minimum of 2 years’ experience as an APC preferred. Any chronic care management is an additional bonus but not a requirement.
This is a salary position plus bonuses
