Job Summary
We are looking for a Intake Coordinator Assistant to join our team! The Intake Coordinator Assistant assists the agency in the intake of new patients and management of physician orders under the direction of the Director of Nursing / Clinical Supervisor. The coordinator will be responsible for ensuring the Agency has accurate physician information on file, physician orders are sent to physicians and returned with signatures timely through verbal calls, electronic faxing, and manual delivery, as needed. The Intake Coordinator Assistant shall assist in the tracking and logging of new patient referrals and ensure agency maintains accurate insurance information on file through routine eligibility verification. The Intake Coordinator Assistant shall function as an Intake Coordinator / Orders Manager in a back-up capacity and/or in their absence, to include greeting incoming patients, recording their personal details, gathering medical histories, and obtaining/verifying any health insurance information/eligibility that they have. The coordinator will communicate with referring facilities and providers and assist in assigning clinicians to meet patient care needs. The coordinator will also manage orders and EHR to facility continuity of care.
Essential Functions:
Ensuring the agency has accurate physician information for active and newly admitted patient.
Assisting in the tracking / logging of new patient referrals on Agency referral log.
Ensuring physician orders are sent to physicians and returned with signatures timely through verbal calls, electronic faxing, and manual delivery, as needed.
Management of physician orders and medical records to ensure timely signing and uploading into EMR system.
Verification of Insurance eligibility and services prior to initiating care and routinely thereafter.
Communicate with patients, physicians and the agency to properly coordinate home health services and insurance requirements, as needed.
Assist in the scheduling of appropriately skilled clinicians to fill patient needs, input scheduling information on the computer and continues to assist in the coordination of scheduling.
Schedule replacement workers as needed and input scheduling information into EMR system.
Operate as an Intake Coordinator / Orders Manager in a back-up capacity, in the absence of.
Complete all required trainings, meetings and in-service requirements. Prepare reports for required meetings.
Perform other duties as directed by Director of Nursing/Administration.
Competencies/Educational Requirements:
Experience in a Home Health Care setting, preferred.
Must be computer literate and able to maintain simple records in English.
Must be able to pass Level II background and OIG/NSO clearance.

It requires minimal lifting of office records and printouts.

Candidate with prior experience in a healthcare office strongly preferred.
Ideal candidate will have prior experience as a Medical Assistant, Office Manager or Assistant in a healthcare setting.
