    Job Description

    JOB DESCRIPTION​ Saint Luke’s Physician Group has a new unique opportunity in their Population Health Division for an experienced worker who has experience with outpatient pharmacy that is looking to grow in a new position with high visibility.  This position would allow you to work with a group of ambulatory care pharmacists and other members of the patient care team within the primary care setting.   

    This position requires you to have pharmacy experience and a desire to deliver excellent patient care.  

    As a population health pharmacy outreach specialist, you will have the opportunity to use your pharmacy background and communication skills to help optimize medication use for our patient population by improving healthcare and medication access. This plays an integral role in the quality of care we are able to provide our patients.     The hours for this position are Monday-Friday, 8:00 am-5:00 pm.

    Support the ambulatory pharmacist staff in care coordination for high risk patients and outreach to patients to schedule appointments for routine annual exams and close gaps in care.  

    • Manage the population health pharmacy medication management referral queue.
    • Schedule new patient and follow up appointments for patients referred to the population health pharmacists.
    • Assist patients in overcoming medication affordability issues by communicating with prescription insurance plans, completing prior authorizations, enrolling patients in manufacturer patient assistance programs, and investigating prescription benefits (ex. prescription test claims) to determine medication costs.
    • Employ exemplary customer service skills in all patient and caregiver interactions.
    • Effectively and respectfully communicate verbally and written in the electronic medical record with patients and all members of the patient care team.
    • Obtain accurate medication lists from a variety of sources including patients, caregivers, retail pharmacies, and insurance claims.
    • Utilize pharmacy knowledge and problem-solving skills to identify and solve medication access and adherence concerns for patients.
    • Assist in connecting patients to appropriate medical and community resources.
    • Participate in organizational and departmental performance improvement efforts in conjunction with pharmacists, including improving healthcare quality measures for our patient population
    • Sending individual and batch messages to patients via the secure patient portal (MyChart) and mailing patient reminders and contact letters

    In this position you will be working as a team in coordination with small group of pharmacists.  To be successful in this role you will need to be able to effectively communicate with patients and healthcare providers, most frequently via telephone and in the electronic medical record. To be successful in this role you will need to be able to manage your own time and workload independently. You should have a solid understanding and interest in improving mediation and healthcare access. This employee should have a vested interest in the overall goals of effective population health management and value-based care to ensure you are supporting the Population Health program for the Saint Luke’s Physician Group Primary Care providers and clinics.
