    RESPONSIBILITIES: 

    • Inform DNS, Medical Director and the Infection Control Committee/QAA Committee of problems and progress in preventing and controlling infections; act as liaison between all departments; ancillary services and the Infection Control Committee/QAA

     

    Committee; advise care center personnel of isolation policy for disease-specific precautions for residents with infection; act as a liaison with the local health department in reporting infectious diseases in the care center and make recommendations to the Infection Prevention and Control/QAPI Committee.  Maintain close communication with all supervisors and nurses and ensure that he/she is the primary focal point for accumulation of information related to possible communicable disease when a resident is admitted or infection is acquired in the care center 

    • Complete an annual Infection Prevention/Control risk assessment/surveillance plan based on the population served, services provided and analysis of surveillance data, identify opportunities for improvement and oversee improvement activities     
    • Consult in the development/revision and implementation of the Infection Prevention/Control program, evaluate it’s effectiveness and modify as needed    
    • Attend the Infection Control Committee/QAPI Committee meetings and coordinate the implementation of committee recommendations; compile data related to the care center infections, infection prevention measures, antibiotic stewardship and flu/pneumonia immunization according to the Infection Control policy and procedure and report to the Infection Control/QAPI Committee 
    • Promote performance improvement.  Review and assist with development/revision of policies and procedures focused on performance improvement.  Ensure that resident needs and rights are considered in development of processes.      
    • Review all infections acquired and nosocomial infections monthly and quarterly, investigate unusual epidemics, clusters of infections and/or infections due to unusual pathogens, make care center rounds to ensure techniques and procedures are performed in accordance with standards set by the QAPI Committee according to the Infection Control Policy and Procedure Manual  
    • Monitor antibiotic use and promote antibiotic stewardship.  Review antibiotic stewardship and report findings/trends to the QAPI committee.
    • Monitor flu and pneumococcal immunization status and promote immunization.  Review flu & pneumonia immunization and report findings/trends to the QAPI committee.
    • Oversee surveillance system, process and outcome measures using epidemiological principles with routine, on-going and systemic collection, analysis, interpretation and sharing of data.  Evaluate & modify the surveillance plan as necessary.
    • Provide input and assist in the development/revision of infection prevention/control policies and protocols and communicate infection prevention/control information and data to committees within the care center and health care personnel as appropriate
    • Train care center personnel to complete Infection Surveillance Reports and supervise follow-up activities; assist with programs regarding infection control and prevention, antibiotic stewardship and assist in the development and/or implementation of infection prevention and control measures 
    • Assess learning needs of residents and staff.  Develop/provide established training based on educational goals, objectives, strategies to promote learning including using available educational tools and technology.  
    • Conduct training for residents, health care personnel, environmental services, maintenance, dietary staff, volunteers and others who enter the care center on a regular basis.  
    • Educate staff regarding infection prevention and control including, but not limited to hand hygiene, respiratory hygiene/cough etiquette, donning/doffing PPE, transmission based precautions and PPE utilized, antibiotic stewardship, cleaning/disinfecting patient care equipment, safe medication administration and proper handling of linen.
    • Conduct adherence monitoring to provide feedback to health care personnel and reinforce training concepts.  Evaluate the effectiveness of educational programs and modify as needed.
    • Ensure compliance with regulatory and mandatory reporting requirements at the local, state and federal levels.  Ensure compliance with COVID-19 and other testing requirements.  Monitor and facilitate compliance with regulatory standards among residents and health care personnel.
    • Provide consultation to occupational health personnel related to infection prevention/control policies, risk of occupational exposures to infectious disease, rates and trends of occupational exposures, work restrictions/recommendations related to communicable diseases or following an exposure.  Assist with immunization and screening programs.
    • Other duties as assigned
    • • Comply with, support and enforce all Company policies related to safety and infection control procedures including the proper use of mechanical lifts, gait belts and personal protective back supports 
    • Understand, comply with and promote all rules and regulations regarding residents’ rights; promote positive relationships with residents, visitors, and regulators, to include presenting a professional appearance

    QUALIFICATIONS:

    • Must be a Licensed Practical Nurse (LPN), Licensed Vocational Nurse (LVN), or Registered Nurse (RN) in good standing and currently licensed by the State 
    • Must have completed specialized Infection Preventionist training or be willing to complete within 60 days of hire, or if Infection Preventionist is a current employee, training must be completed within 30 days of beginning the role (minimum of 14 hours). 
    • Must complete a minimum of 10 hours of continuing education in infection prevention/control annually.
    • Must have experience in clinical nursing 
    • Must be able to apply statistical methods for documenting infection rates
    • Must be capable of maintaining regular attendance
