    Job Title: Special Needs Coordinator.

    Based at: Long Beach,CA.

     

    JOB SUMMARY:  SANFORD FEDERAL is looking for a successful SPECIAL NEEDS COORDINATOR WHO WILL establishes and maintains procedures to identify sponsors whose family members have special medical and/or educational needs in a timely manner.

     

    • REQUIREMENTS:  The Candidate must meet the following requirements

    • EXPERIENCE

    •  Must have a minimum of two (2) years of a Bachelor of Science in Nursing (BSN) with full-time experience in specialized field of study and care coordination; as well as one (1) year of clinical services being performed in the last three (3) years.

    • Must have the knowledge and skills to effectively apply SNC functions.

    • Must have the ability to identify patients and families with special medical and educational needs.

    • EDUCATION

    • Graduate of a baccalaureate degree program in nursing (BSN), accredited by a national nursing accrediting agency recognized by the US Department of Education.

    • Maintenance of Basic Life Support (BLS) certification is mandatory.

    • LICENSURE

    • Must have an active, valid, current and unrestricted license (with no limitations, stipulations or pending adverse actions) to practice nursing as a registered nurse in any US state/jurisdiction.

    • DUTIES: the candidate will be responsible for,

    •  Coordinates with MTF TRICARE representatives to provide information to beneficiaries about community, state and national resources specific to the special need’s population.

    • Provides AFMRA Chief of Medical Staff the MTF/Commanders-signed appointment letters for the Chief of Medical Staff, SNC and alternates for access to password-protected data systems.

    • Ensures every RegAF sponsor with one or more family members with special needs that is assigned to the installation has an Special Needs (SN) file maintained at the MTF and a case established in the AF-provided special needs data system (Q-base).

    • Ensures all Army, Navy, and Marine Corps sponsors of family members with special needs are referred to their Services’ EFMP point of contact(s) for enrollment and relocation support IAW Service specific guidance.
