Shift: Full-Time, Days (Rotating Weekends)

Salary:

• Social Worker 2 (SW2): $24.75 - $34.65 per hour
• Social Worker 3 (SW3): $29.41 - $41.16 per hour
  Sign-On Bonus: Up to $10,000

Job Summary

We are seeking a dedicated Social Worker (MSW) to provide non-clinical biopsychosocial assessments and interventions for our inpatient floors and outpatient emergency department. The MSW will perform crisis intervention, patient and family interventions, high-risk screening, focused emotional support, and referrals for financial assistance or other resource needs. This role involves working with intensive care units, Trauma, Oncology, NICU, Transplant, and ED.

Key Responsibilities

• Collaborate with RN case managers, acute care nurses, physicians, insurance companies, and other staff to identify and address patient needs.
• Identify and address barriers to discharge and recovery, including financial, emotional, psychosocial, medical, and spiritual barriers.
• Maintain thorough knowledge of Medicare, Medicaid, and private payer regulations and processes, and understand managed-care concepts and length-of-stay management.
• Perform thorough assessments and develop appropriate discharge plans for high-risk patients with discharge planning or psychosocial needs.
• Coordinate and facilitate meetings between the care team and patients/families/caregivers, assisting in making difficult decisions while honoring their preferences and values.
• Provide emotional support to patients and family members during complex issues such as end-of-life, terminal illness notifications, death notifications (with physician), and crisis interventions.
• Engage in consultation, discharge planning, education, counseling, advocacy, and information referrals to meet the emotional, social, and financial aspects of care.

Qualifications

• Education: Must have a Master's Degree in Social Work from an accredited school.
• Licensure: State license eligible.
• Experience: Minimum of 1 year of hospital Social Work experience preferred.

  • Social Worker 2 (SW2): Must have LMSW; RPC will offer clinical supervision.
  • Social Worker 3 (SW3): Must have LCSW license.
