    Seabury Resources for Aging is a private, not-for-profit corporation registered in the District of Columbia, which provides various services to enhance the quality of life for older adults. Seabury’s mission is to provide personalized, affordable services and housing options to help older adults in the greater Washington, DC area live with independence and dignity.     Overview: The Contract Care Manager is responsible for providing care management services to Seabury Care Management clients. This position requires significant time working with care management clients through home visits, and requires detailed timely documentation with regard to client records, care plans and service notes. The Contract Care Manager reports to Care Management Supervisor. The Care Manager works from a home office and will need to use a computer, enter client service notes into Seabury database, provide her/his own internet service, cell phone and transportation/car.

    Major Responsibilities/Activities:

    Completes client assessments, care plans and provides on-going monitoring as stated in each individual client’s contract

    Meets care management service unit targets based on agreed upon commitment of work time. Full time or Part time (50% or more) position available.

    Administrative tasks expected with those service units including:

    Inputs service notes into database for each client within 48 hours of contact with, or completion of work on behalf of client

    Edits service notes for each client served by the 3rd day of the following month

    Maintain personal client files and keep organized

    As requested, provides outreach and networking to inform the community of Care Management services

    As requested, assists other care managers with their cases

    Informs the Care Management team about new services and resources in the community via email and attendance at team meetings

    Attends networking events as requested.

    Provides weekend coverage of phones and emergencies on a rotating basis with other administrative staff and care managers. (6 times per year)

    Provides emergency and vacation coverage for other staff

    Performs other duties as needed

    Minimum Qualifications:

    Master’s degree-prepared social worker currently licensed in the District of Columbia and/or Maryland, with ability to maintain licenses (clinical license preferred), or; Master’s degree in related field with accreditation from, and membership in the Association of Aging Life Care Professionals.

    Preferred experience working with older adults and their caregivers, with solid references acknowledging skills and effectiveness.

    Working knowledge of Microsoft Office applications; ability to learn case management software, database, etc.

    Excellent oral, written and interpersonal communication skills; effective email and Internet skills.

    Possessing strong organizational skills, good record keeping habits, and ability to set priorities and meet goals.

    Solid knowledge and experience with aging issues and local resources, elderly clients, and trends in social work.

    Have means and ability to travel primarily within the District of Columbia, lower Montgomery County. (We also have clients in Prince George’s Counties, MD and Northern Virginia. Though this position is primarily for DC and Montgomery County, there may be opportunities elsewhere. )

    Good problem solver who is able to analyze problems and make recommendations to solve them.

    Maintain liability insurance, valid driver’s license and car insurance.

    Self-starter and leader.

    Work Environment: Home office, frequent local travel to client’s homes.

    Essential Physical Functions: Ability to lift 20 pounds; ability to drive a vehicle (along with possession of a driver’s license) and ability to sit for long periods of time.

    Equipment Used: Contractors are expected to have their own PC/laptop, internet, Smartphone with the ability to talk, text and check email while in field.
