    The mission of Activate Healthcare is to transform health care by empowering people to take charge of their health and employers and families to take charge of health care costs. The Health Promotion Coach position will play an important role in bringing that mission to life by empowering employees and spouses to care for themselves through effective remote behavior change interactions.

    This role is dedicated to working with the health promotion team for a specific client to engage participants in lifestyle behavior change through one on one health coaching, supporting well-being and culture change initiatives, and supporting population communication and education.  This is a full time position based in Indianapolis, Indiana.

    RESPONSIBILITIES

    The Health Promotion Coach will work within the dedicated client-specific team to engage employees and spouses in lifestyle behavior change through one on one coaching.

    • Engage employees and spouses in lifestyle behavior change, including:

      • Individual, personalized coaching to participants to support healthy lifestyle behavior change
      • Encouraging employees to state values through their motivation statement
      • Assessing opportunities for change through health profile review
      • Communicate with participants in manner to increase rapport and engagement
      • Setting collaborative goals to engage ownership
      • Monitoring goals and communicating between appointments
      • Measuring progress, individually and across the population
      • Seeking ongoing opportunities to learn, refine and integrate effective coaching methods
      • Maintaining accurate records and ensuring confidentiality of all data
      • Collaborate with the client-dedicated team to continuously improve the coaching experience of participants
    • Optimize technology to support lifestyle behavior change, including:

      • Ensuring confidence and effectively and efficiently using the tool
      • Instructing participants in downloading and getting started with the tool
      • Facilitate continued use of the tool for tracking progress, communications, and reward updates
      • Staying abreast of updates and providing feedback for enhancements
      • Assist with enhancements, trouble shooting, or testing needed for reward system
      • Communicating with customer support on behalf of participants, as necessary
      • Granting rewards earned through a variety of wellness activities
      • Track individual participants to demonstrate goal achievement, outcomes and results
    • The Health Promotion Coach will support health promotion initiatives to promote well-being and culture change.

      • Assist with identifying population needs, interests and expectations
        • Focus on areas supported by coaching interactions, screening and health profile data in “MEDSS” areas, i.e. move more, eat and drink healthy, don’t use tobacco, stress well, and sleep soundly
        • Assist with creating communication materials and promotional items to encourage participant engagement
        • Offer guidance to participants regarding program details to ensure a positive experience.
        • As needed, monitor the client-dedicated phone line and in-coming messages.

    Required Skills

    • Positive attitude and a strong “servant leadership” mentality to all internal and external customers.
    • Ability to work with speed and agility, including managing coaching schedule during high volume seasons.
    • Experience communicating and working with customers and stakeholders.

    Required Experience

    EXPERIENCE

    • Three or more years related industry experience
    • Three or more years health coaching experience
    • Bachelor’s degree or higher in a health-related field (i.e. Health Promotion, Exercise Physiology, Nutrition, Nursing, Public Health, Health Psychology) from an accredited college or university program
    • Outstanding knowledge of effective lifestyle behavior change such as tobacco cessation and weight management, and the role lifestyle plays in conditions such as diabetes and heart disease
    • Demonstrated experience applying health behavior change theories and principles, such as the Transtheoretical Model and Motivational Interviewing

    PREFERRED

    • Behavior change coaching certification, such as Well Coaches or Intrinsic Coaching
    • CHES, ACSM or other fitness certification
