Required Skills and Experience:

1. California CNA or MA certification

2. Interest in working with individuals with possible dementia and/or mental health diagnoses

3. At least 1 year of experience working in the healthcare industry

4. Strong communicator with the ability to engage with various stakeholders in a professional and warm manner

5. Empathy and a genuine interest in providing care for the geriatric population

What You Need to Know:

1. This role requires travel to and from multiple facilities in the area and so demonstrated safety while driving is important to us. The company will reimburse for mileage to and from the facilities*

2. Territories include:

a. Mission Viejo and Laguna Beach

b. Yorba Linda and Whittier

Additional Job Details:

1. Conduct mental health and cognitive assessments such as depression and anxiety screenings and basic memory testing, obtain accurate vitals, know how to populate certain sections of charts such as problem lists and medications.

2. Troubleshoot any technical issues, including a basic understanding of Apple products and software applications

3. Identify communication issues and work with patients and providers if rescheduling is needed

4. Communicate and collaborate with the clinical team
