JOB DUTIES:

1. Welcome and screen employees, visitors and staff using a health and travel history declaration form/questionnaire

2. Operate infra-red thermometer or other assigned instrument to obtain a temperature reading of all individuals entering the facility according to the manufacturer's instructions and CDC guidance for personal protection

3. Follow CDC guidelines to determine if an individual is clear to enter the facility

4. Reporting cases of positive and negative responses according to established protocols while remaining cognizant of appropriate requirements (state, local and federal requirements established per site)

5. Provide additional information to employees, patients or visitors once responses have been documented

6.. Ensure individuals maintain appropriate social distancing of 6 feet as recommended by the CDC during the health screening process Maintain cleanliness of equipment and work area

EDUCATION/ WORK REQUIREMENTS:

1. High School Diploma or GED required

2. Pleasant personality with good interpersonal skills

3. Appropriate written and verbal communication skills

4. Strong team player and ability to work independently

5. Healthcare, Customer Service, Retail, Hospitality... EXPERIENCE ACCEPTED
