    Position Overview:

    We’re hiring a dynamic Service Coordination Supervisor to lead a fast-growing team providing care coordination services to vulnerable populations. We're looking for someone with a healthcare operations or care management background who can bring structure, leadership, and accountability to a dedicated and expanding team.

    This role is essential to our next stage of growth, with the opportunity to implement a team lead model, refine service coordination practices, and ensure compliance and quality across the board. We currently have a strong foundation, including administrative and compliance support, and are adding 1–2 new coordinators per month.

    What We’re Looking For:

    • experience in healthcare management, care coordination, case management, or a related field

    • Strong operational leadership with the ability to build systems, delegate effectively, and foster staff retention

    • Skilled at managing remote and diverse teams, ensuring consistency, communication, and follow-through

    • Patient and detail-oriented, especially when reviewing service plans, documentation, and compliance

    • Comfortable training, mentoring, and holding both staff and team leads accountable

    • Tech-savvy and organized, with the ability to streamline workflows and implement scalable solutions

    • A proactive problem-solver with strong critical thinking and communication skills

    • Committed to delivering high-quality services with a focus on team culture, retention, and long-term success

      Key Responsibilities:

    • Supervise and support a team of 13–15+ Service Coordinators, adding new staff monthly

    • Implement a team lead model to manage scale and maintain oversight

    • Review documentation for accuracy and ensure ongoing regulatory compliance

    • Promote team retention through a supportive and accountable leadership style
    • Coordinate with operations, admin, and compliance to enhance internal systems

    • Handle client and family relations, addressing escalated concerns as needed

    • Explore and evaluate new services or operational improvements as the organization grows

    • Maintain a structured, mission-driven team culture rooted in quality and consistency
