Service Coordination Supervisor – Healthcare Management

Position Overview:
We’re hiring a dynamic Service Coordination Supervisor to lead a fast-growing team providing care coordination services to vulnerable populations. We're looking for someone with a healthcare operations or care management background who can bring structure, leadership, and accountability to a dedicated and expanding team.
This role is essential to our next stage of growth, with the opportunity to implement a team lead model, refine service coordination practices, and ensure compliance and quality across the board. We currently have a strong foundation, including administrative and compliance support, and are adding 1–2 new coordinators per month.
What We’re Looking For:
experience in healthcare management, care coordination, case management, or a related field
Strong operational leadership with the ability to build systems, delegate effectively, and foster staff retention
Skilled at managing remote and diverse teams, ensuring consistency, communication, and follow-through
Patient and detail-oriented, especially when reviewing service plans, documentation, and compliance
Comfortable training, mentoring, and holding both staff and team leads accountable
Tech-savvy and organized, with the ability to streamline workflows and implement scalable solutions
A proactive problem-solver with strong critical thinking and communication skills
Committed to delivering high-quality services with a focus on team culture, retention, and long-term success
Key Responsibilities:
Supervise and support a team of 13–15+ Service Coordinators, adding new staff monthly
Implement a team lead model to manage scale and maintain oversight
Review documentation for accuracy and ensure ongoing regulatory compliance
Coordinate with operations, admin, and compliance to enhance internal systems
Handle client and family relations, addressing escalated concerns as needed
Explore and evaluate new services or operational improvements as the organization grows
Maintain a structured, mission-driven team culture rooted in quality and consistency
