  • Responsibilities

    Participates and Collaborates in the Patients Plan of Care with other ordered Disciplines to meet goals

    Implements the Care as directed on the CNA assignment sheet

    Possess and develops a skill set as appropriate and allowable for a Certified Nurses Aide

    Reports abnormal situations or patient symptoms to the RN Case manager or Clinical Coordinator

    Has the ability to take direction and offer input to Professional disciplines involved in case or Home Health management

    Demonstrates a consistent efficient level of performance in Scheduling/ completing assigned visits and required documentation timeframes and meeting productivity parameters

    Demonstrates a consistent efficient level employment requirements and departmental processes.

    Maintains consistent timely communications i.e hand off communication, Departmental postings, Management requests and meetings.

    Required Skills

    MINIMUM EDUCATION: High School Graduate or Equivalent

    REQUIRED LICENSES: Nursing Aides -Must hold a Certification in Nurse's Aide training meeting the Federal requirements.-Must possess a valid Driver's License with proof of Automobile insurance.

    Certified Nurse Assistant with the state of Kentucky.

    BLS certification – American Heart Association (Healthcare Provider) or American Red Cross (Healthcare Provider) required, must be current.

    Required Experience

    MINIMUM WORK EXPERIENCE: Must be able to transport self in inclement weather. Must possess a reliable maintained representative vehicle. At least 1 year experience required. Home Health Experience preferred. Must possess computer / typing skills. Excellent communication skills. Ability to tolerate a high stress environment.
