PAY RANGE: $15.00 to $17.00 per hour

POSITION SUMMARY
As a provider for day program and SCC services, clients will be assisted with the acquisition, retention or improvement in self-help, socialization and adaptive skills that takes place in a non-residential setting, separate from the participant’s private residence or other residential living arrangement, except when due to medical and/or safety needs. The environments that the client will be in are to foster the acquisition of skills, appropriate behavior, greater independence, and personal choice. These services are individually coordinated through the client’s Service Plan.
ESSENTIAL DUTIES/RESPONSIBILITIES
- Provide supports that allow ability and skill development necessary to enable the client to access typical activities and functions of community life such as those chosen by the general population (including, but not limited to community education or training, retirement and volunteer activities).
- Provide a wide variety of opportunities to facilitate and build relationships and natural supports in the community.
- Utilize the community as a teaching environment for clients for services and supports identified in the individual’s service plan.
- Services/activities will be conducted in a variety of settings in which clients will interact with non-disabled individuals (other than staff).
- Services will include socialization, adaptive skills and personnel to accompany and support clients in the community, resources necessary provided for participation in activities and supplies related to skill acquisition.
- Services will be provided to 2-6 clients on a daily basis.
- Complete client specific training as needed.
- Follow all protocols set forth for each client receiving services.
- Log noting will be completed in Therap weekly at a minimum.
SKILLS & ABILITIES
- Must have the ability to communicate effectively utilizing forms, reports and follow verbal or written instructions.
- Ability to follow guidelines of services outlined by State requirements and in accordance with a client’s services plan.
- Interpersonal skills needed to effectively interact with a person with developmental disabilities.
REQUIREMENTS
- Available for providers, relatives and legal guardians.
- Must be at least 18 years of age.
- Must be willing to take and pass CAPs and background checks.
- Must complete all employee training requirements within 90 days of employment.
- Must be in good physical, mental and emotional health.
- Must be able to lift a minimum of 50lbs, if required.
- Must have valid driver’s license and a good driving record.
- Must have a vehicle in good working order with current insurance and registration.
- Ability to work with a variety of technology including cell phones and Laptops.
- Willing to stay connected to co-workers electronically.
- Working knowledge of internet, Windows 10, Skype, email, Microsoft Office programs and other data base programs.
- CPR/First Aid and QMAP certified (or ability to complete certifications within 90 days of employment.
DESIRED QUALIFICATIONS
MATERIAL AND EQUIPMENT DIRECTLY USED:
- Computers, Company Vehicle, Personal Vehicle, Laptops, Cell phones, etc.
- SCC Sites or Community Areas.
- Operation of company vehicles.
- Moderate physical activity required.
- Frequent sitting and standing.
- A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required.
