POSITION SUMMARY
The Waiver Services Coordinator is responsible for supervising all aspects and all services provided by the SLS, DD, CHRP and CES waivers (excluding residential services), including daily operations, and all persons receiving services. The Waiver Services Coordinator will be an active part of Waiver Services development, performance management, policy recommendation, policy creation and policy implementation. The Waiver Services Coordinator will be considered a Case Manager and thus must remain responsive, agile and dedicated to the needs of the individuals we serve at Shiva Enterprises.
ESSENTIAL DUTIES/RESPONSIBILITIES
- Oversee all Waiver Services (excluding residential services).
- Oversee Direct Care Providers, and Clients on services to ensure programming is meeting established criteria.
- Ensures communication at all levels is being utilized appropriately and effectively between employees, contractors, caregivers and clients.
- Responsible for maintaining and submitting regularly scheduled reports as required by the Department of Health Care Policy and Financing (HCPF), including but not limited to quarterly monitorings.
- Revise program documents, processes, and policies as needed to increase efficiency and comply with changing regulations, per rules and regulations established by Health Care Policy and Financing (HCPF).
- Responsible for responding to referrals for services and the enrollment of new individuals into specified programs.
- Main point of contact to answer questions regarding Waiver Services. Facilitate enrollment meetings, meet and greets to introduce providers, potential persons receiving services, families, and their interdisciplinary team.
- Responsible for maintaining independent contractors, employees, and client files, ensuring all paperwork meets rules and regulations established by Health Care Policy and Financing (HCPF), with a caseload of up to 20 to 30 clients.
- Ensure all Waiver Services contractors and employees are maintaining the proper training and certifications required by Health Care Policy and Financing.
- Review program paperwork as necessary, including ISSPs, BISSPs, incident reports, etc., as well as identifying trends in documentation.
- Attend client’s IDT’s, annual staffing’s, IEP meetings, HRC reviews, and other meetings, as needed and complete meeting notes when required.
- Implement and mediate informal and formal conflict resolutions. Including but not limited to, addressing, and working to resolve any questions, comments, and complaints submitted by independent contractors, Shiva employees, and clients.
- Attend, contribute, and participate in supervisor meetings to advance the agency, persons receiving services, and employees’ potential and opportunities.
- Maintain current knowledge about all Waiver Services and assure ongoing compliance with Rules and Regulations and related practices by attending Health Care Policy and Financing required trainings.
- Initiate and conduct MANE investigations as needed following Shiva Enterprises LLC’s established policy and procedure.
- Network with other professionals, community developers, and local leadership to be aware and informed of the range of options available to meet the needs of all community members.
- Participate in disseminating information up and down the chain of command.
- Other duties or projects as assigned.
REQUIREMENTS
- Must be willing to take and pass CAPs and background checks.
- Must be in good physical, mental and emotional health.
- Must be able to lift a minimum of 50lbs, if required.
- Must have valid driver’s license and a good driving record.
- Must have a vehicle in good working order with current insurance and registration.
- Ability to work with a variety of technology including cell phones and Surface Pros/Laptops.
- Willing to stay connected to co-workers electronically.
- Working knowledge of internet, Windows 10, Microsoft Teams, Microsoft Outlook, Microsoft Office programs and other data base programs.
- Must have a bachelor’s degree in Human Services, Social Work, Education, Psychology or a related field and one year of successful experience in Human Services.
- Or, an Associates Degree from an accredited college and two years of successful experience in human services.
- Or, four years of successful experience in Human Services.
- 6 months experience working in case management.
- 1 year or more of experience working with IDD Medicaid Waivers in the State of Colorado.
DESIRED QUALIFICATIONS
- 2 years or more of experience working with IDD Medicaid Waivers in the State of Colorado.
- 1 year or more of experience working in case management.

WORKING ENVIRONMENT/PHYSICAL ACTIVITIES:
- Office and waiver service sites.
- Occasional remote work from home.
- Moderate physical activity required.
- Frequent sitting and standing.
- A working vehicle, valid CO driver’s license (unless active Military family), and minimum insurance levels ($100,000/$300,000/$100,000 liability coverage) are required.
