    This role reports directly to our Senior Vice President of Customer Experience and is a critical part of Epicured’s New York Medicaid Section 1115 Waiver program, supporting Medicaid members through screening, navigation, and connection to eligible services.

    Role Overview

    Epicured is seeking Social Care Navigators to support our expanding New York Medicaid Section 1115 Waiver initiative. This program is designed to improve health outcomes, expand equitable access to care, and reduce health disparities by helping Medicaid members receive support for health-related social needs (HRSN) and qualifying nutrition-related services.

    In this role, you will conduct outreach and screening with Medicaid members telephonically to assess eligibility for Epicured services and help navigate members to internal and external resources they qualify for. You will use designated contact center and program platforms to accurately document screening outcomes, referrals, and next steps.

    This position is ideal for someone who is highly organized, compassionate, tech-savvy, comfortable in a high-volume call environment, and motivated by supporting members with complex needs.

    Key Responsibilities

    • Conduct high-volume outreach and screening for Medicaid members under the NY Section 1115 Waiver , including assessing eligibility and identifying appropriate next steps

    • Utilize a virtual contact center platform ( Five9 ) to manage inbound/outbound member interactions and meet daily activity expectations

    • Complete member screening workflows and navigate eligible members to Epicured nutrition services and other internal/external support resources

    • Document screening outcomes, member eligibility status, outreach attempts, case notes, and referral outcomes accurately within required technology platforms

    • Support members by providing clear and accurate information related to services they may qualify for and how to access them

    • Collaborate with internal teams and/or program partners to ensure smooth handoffs and follow-through for members with complex needs

    • Maintain high attention to detail, confidentiality (HIPAA), and adherence to established workflows and compliance standards

    • Attend scheduled trainings, meetings, and performance check-ins as required

    • Perform additional duties as assigned in support of program operations and member success

    Qualifications

    • High School Diploma, HSE, or GED required

    • Experience with high-volume outbound calls, member outreach, customer service, patient intake, or call center environments

    • Working knowledge of Medicaid, healthcare benefits, and/or social care services (preferred)

    • Familiarity with community resources and social service programs to support members with health-related social needs (preferred)

    • Ability to work collaboratively in a team environment while also managing independent workflows

    • Proficiency in operating computer software and comfort using technology platforms for documentation and tracking

    Schedule Requirement:

    • Ability to work 9:00 AM – 6:00 PM EST, Monday – Saturday (specific schedule assigned based on coverage needs)

    Preferred Qualifications

    • Healthcare, patient screening, member services, care coordination, navigation, or community-based program experience

    • Experience supporting members with complex needs through structured screening and referral processes

    • Multilingual candidates strongly encouraged to apply, including: Spanish, French, Hindi, Georgian, Mandarin Chinese, Korean

    • Familiarity with Social Care Network (SCN) platforms or workflows (highly preferred)

    Compensation & Benefits

    • Salary Range: $20–$22/hour, commensurate with experience
