JOB TITLE: Social Service Coordinator
POSITION REPORTS TO: Senior Center Manager/Satellite Coordinator

Primary Purpose of Job
The primary purpose of this position is to be responsible for the direct and indirect provision of social services offered at the senior center.
Essential Job Functions
· Provide comprehensive social services to center participants and consumers living in the community. This may include but is not limited to, information and referral, intake, assessment, case planning, and follow up.
In special circumstances, when indicated, make home visits to check on the participant’s status, follow up on family issues, or determine appropriateness for center participation.
· Obtain information to determine consumer eligibility and assist in making application for entitlement programs and benefits.
· Directly provide, or coordinate, registration of center consumers and assessment of strengths, social service needs (i.e., nutrition/food insecurity, transportation, etc), and other circumstances which may facilitate or impede healthy aging.
· Provides supportive individual or group counseling to consumers and their families.
· Evaluates consumers for assistance with transportation and completes required documentation to facilitate services.
· Maintains and updates consumer files, contact/attendance logs, and supporting documentation in compliance with CHCS and PCA requirements and professional standards.
· Utilize PCA Client tracking database, COMPASS, and other software to facilitate work.
· Develops and maintains a resource file for referring consumers to services in the community.
· Coordinates the provision of social services with the Senior Center Manager and Program Coordinator. This may include but it not limited to mental health services, legal services, registration for and receipt of supplemental food programs, transportation. etc.
Provision of services may occur through programming at the center, virtual/online services to consumers, phone contact with consumers (e.g., wellness calls), and other means as appropriate.
· Prepare and distribute materials (e.g., flyers, newsletter pages, posters) to inform consumers about the availability of services, benefits, etc. relevant to their circumstances.
· Recruit, train, and supervise senior center volunteers and student interns, as assigned by the Senior Center Manager.
· Develop and maintain a good working rapport with interdepartmental personnel, other departments and partners of the program to assure that all services and activities can be adequately maintained to meet the needs of consumers.
· Attend, participate in, and complete continuing educational programs designed to keep the Social Service Coordinator abreast of new resources, providers, services, and changes in the profession.
· Comply with current senior center operational requirements as outlined by CHCS, PCA, the State Department on Aging, local Department of Health, and other relevant stakeholders.

· Minimum of a Bachelor’s Degree in Social Work or related field.
