As the Social Service Coordinator , you’ll work alongside our housing team and supportive services department to help senior residents navigate everything from Medicaid re-enrollment to home care referrals, from pest control challenges to emotional wellness check-ins. Your mission: ensure no resident falls through the cracks.
You won’t be doing this work alone. You’ll be part of a collaborative team that includes property managers, maintenance staff, and community partners—all committed to helping our residents feel seen, safe, and supported. You’ll be empowered to take initiative, organize services across two locations, and most importantly, build meaningful relationships with the seniors we serve.
A Day in the Life
Each day, you'll work out of either New Life Manor (M/W/F) or Byrne Court (T/Th) , providing on-site coordination and follow-through for residents' support needs. You’ll offer one-on-one support for seniors navigating insurance renewals, benefits applications, home care access, or just needing someone to check in. Whether it's coordinating a blood pressure screening with a partner agency or knocking on a neighbor’s door to make sure they’re okay, you’ll be the go-to person for consistent, compassionate care.
You’ll maintain and track attendance logs, help build and maintain a monthly calendar of enrichment events, and ensure both sites have equitable access to resources and partnerships. At least once a week, you’ll meet with the broader housing team, and once a month, you’ll sit down individually with your supervisor for dedicated support. You’ll also play a key role in collaborating with property managers around issues like housekeeping support, lease compliance, and navigating behavioral or hygiene challenges with sensitivity and accountability.
This role is equal parts case management, community coordination , and advocacy —designed for someone who wants to be out from behind a desk, actively engaged with people, and ready to problem-solve in real time.
