    Sidney Regional Medical Center is hiring for a part-time Social Services Representative for our Long Term Care team at Pole Creek Estates. Join our growing team at Pole Creek Estates! This is a part-time opportunity to support approximately 20 hours per week.

    Job Responsibilities:

    • Identifies patients who may need discharge planning services, perform assessment through a review of medical records, and communication with the health care team and patient/family, and determines appropriate discharge planning needs providing patient/resident family with all available healthcare resources and options.
    • Maintains clear and concise documentation in patient record as to assessment, plan, and arrangements that are made and advance directives. Communicate effectively between referral agencies, patient/resident, patient/resident family, physicians, and other staff members of the health care team in evaluation of care to ensure continuity of care. Responsible for completing MDS and signing MDS and Raps and formulating care plan.
    • Maintain up to date information on available Community Hospital resources available.
    • Be an advocate, when applicable to assure that patient/family rights, choices and safety are maintained through the patient's stay and discharge planning process.
    • Lead the telehealth process.
    • May attend the Hospice IDT meeting.
    • Participate with the QAPI reports.
    • Collaborate with the psychotropics.
    • Involve with grievance process.
    • Participate with rounding and hostess activities.
    • Participate in single task feeding as applicable.
    • Maintains reservation/admission accuracy of 97% as applicable.
    • May assist in covering shift regarding non-nursing duties, i.e. making beds, pushing wheelchairs.
    • Assist with Medicare Admissions and Eligibility as applicable.
    • Other duties as assigned by management.

    Education & Experience:

    • Education: Prefer Bachelor's Degree from four-year college or an Associate of Arts Degree in Social or Behavioral Science. Must be willing to complete 36-hour course of instruction by Nebraska Health Care Association.

    • Experience: Have 2 years of experience in a geriatric setting.

    • Required licensure/certification: Attend and pass the Social Worker Class offered by the State of Nebraska. A current and valid Nebraska driver's license is required.

    • Automated External Defibrillator (AED) training.
