    Job Title: Social Worker (Skilled Nursing Facility)

    What You’ll Do:
    As a Social Worker in our skilled nursing community, you’ll be a steady, compassionate presence for residents and their families during some of life’s most challenging transitions. From the moment a resident arrives, you’ll help them feel heard, supported, and connected—advocating for their needs and making sure they never feel like just another patient.

    You’ll work closely with our care team to understand each resident’s story, helping shape care plans that reflect not just their medical needs, but who they are as people. Whether it’s guiding a family through a difficult decision, coordinating a smooth discharge home, or simply being there to listen, your role will make a real, daily impact.

    What You’ll Be Responsible For:

    • Building meaningful relationships with residents and families from day one
    • Completing assessments that help us truly understand each resident’s needs, preferences, and goals
    • Partnering with nursing, therapy, and providers to create thoughtful, personalized care plans
    • Supporting residents and families through emotional conversations, transitions, and challenges
    • Leading care conferences and helping families feel informed and confident in decisions
    • Coordinating discharges—making sure residents leave with the right support and resources in place
    • Helping navigate financial resources, including Medicaid when needed
    • Addressing concerns with empathy, professionalism, and a solutions-focused mindset
    • Advocating every day for dignity, respect, and quality of life
    • Keeping documentation accurate while never losing sight of the human side of care

    What We’re Looking For:

    • Bachelor’s in Social Work (BSW) required
    • Minimum one year experience in healthcare, SNF experience preferred
    • Someone who communicates with empathy, confidence, and clarity
    • A natural advocate who isn’t afraid to speak up for residents
    • Ability to balance multiple priorities while staying present with people
    • A team player who values collaboration and connection
