Compensation: $95,000 - $100,000 Annually

Position Description:

We are looking for an experienced Social Worker ( SW ) for our Skilled Nursing Facility ( SNF ) in the Staten Island, New York area to assist residents who are struggling with mental health issues. This is a full time, in person position Monday - Friday 9am - 5pm.

Qualifications of the Social Worker ( SW ) :

• Valid Social Worker ( SW ) license in New York
• 2+ years of experience as a Social Worker ( SW )
• 2+ years of experience working in a post acute medical setting

Responsibilities of the Social Worker ( SW ) :

• Assess patients through sessions and medical history reviews
• Develop comprehensive treatment plans
• Maintain complete and confidential medical records
