Job Title: Travel Social Worker – Case Management (Patient Centered Services) Location: Washington, DC (Must reside 50+ miles away) Pay Rate: $52–$59/hour Schedule: Monday–Friday, 8:30 AM – 5:00 PM
Position Overview: Patient Centered Services is seeking a skilled Travel Social Worker – Case Management to support patients in an acute rehabilitation setting in Washington, DC. This role is ideal for a licensed clinical social worker with hospital discharge planning experience who is passionate about coordinating care and improving patient outcomes across the continuum.
Key Responsibilities:
Provide social work services to patients, families, and the community in accordance with licensure and regulatory standards
Coordinate and facilitate patient care throughout admission and discharge processes
Conduct comprehensive discharge planning to ensure safe and efficient transitions of care
Collaborate with interdisciplinary teams to support patient needs and care goals
Provide counseling, resource planning, and care coordination in a cost-effective manner
Respond effectively to changing patient needs and work assignments
Document all case management activities accurately using Cerner EMR system
Minimum Qualifications:
Active LICSW (Licensed Independent Clinical Social Worker) certification (Required)
Current Washington, DC Social Worker license (Required)
Minimum 1 year of hospital case management experience, including discharge planning (Required)
Experience with Cerner (Required)

Additional Requirements:
Must reside at least 50 miles from Washington, DC to qualify for travel status
