Social Worker – Home Health Care – Macomb County Territory – 40 Hours/Week – Days

Job Description:

As a Home Health Care Social Worker, you'll work with homebound clients and their families to help them navigate life's challenges and get the most out of their medical care. Under general supervision, you'll implement creative treatment programs that support clients in overcoming personal and environmental obstacles—turning obstacles into opportunities for positive change!

Key Responsibilities:

• Partner with homebound clients and their families to develop personalized treatment plans that address their unique needs and goals
• Implement engaging, practical treatment programs designed to help clients adjust to life changes and maximize the benefits of their medical care
• Provide compassionate support to families navigating difficult transitions and environmental challenges
• Connect clients with community resources and services that enhance their quality of life
• Maintain detailed, organized documentation of client progress and program outcomes
• Collaborate with healthcare teams to ensure coordinated, holistic care for each client
• Build strong, trusting relationships with clients and families through active listening and genuine care

Qualifications

EDUCATION REQUIRED:

• Master's degree in social work required.

EXPERIENCE REQUIRED:

• Requires a minimum of one (1) year of social work experience in a health care setting.

SKILLS REQUIRED:

• Knowledge of the principles of human behavior, individual and group counseling techniques and relevant statutes and regulations.
• Requires very high levels of social sensitivity, personal emotional adjustment and verbal skills in order to provide counseling, assist, and influence others.
• Requires a professional level of analytical skills in order to solve problems, analyze, and develop policies and procedures.

CERTIFICATIONS/LICENSURES REQUIRED:

• Requires Licensure in the State of Michigan as a Social Worker (LMSW).
• Must possess a current State of Michigan driver's license and a satisfactory driving record.
