We are seeking a dedicated and heart-centered Social Worker to join our interdisciplinary team serving older adults and adults with disabilities in a warm, supportive Adult Day Health Care setting.

Job Summary:
Conduct psychosocial assessments and individualized care planning
Provide emotional support, counseling, and crisis intervention
Support participants experiencing isolation, depression, anxiety, grief, or adjustment challenges
Assist families and caregivers with education, resources, and long-term planning
Coordinate with healthcare providers, community agencies, and support services
Participate in interdisciplinary team meetings and participant care conferences
Maintain accurate documentation in compliance with California ADHC regulations
Advocate for participant rights, dignity, safety, and quality of care
Qualifications:
Master's degree in Social Work (MSW) or other related field
or Bachelors degree in Social Work or other related field and 2 years of experience in a healthcare or social services setting
Strong communication and interpersonal skills with a compassionate demeanor.
Ability to work collaboratively in a multidisciplinary team environment.
Knowledge of community resources and services for seniors.
Proficient in documentation and familiarity with electronic health records.
Commitment to ongoing professional development and ethical practice.
Additional Requirements: Current CPR and First Aid certification Bilingual skills are a plus (Vietnamese, Korean)
