Social Worker (MSW or BSW) for our Skilled Nursing Facility (SNF)

Compensation: $80,000 - $95,000 Annual Salary

This is a full time, on-site position. The social worker carries the social work function for the building, covering psychosocial assessment, care planning, discharge planning, and direct support for residents and families, on a caseload that includes substance use and behavioral health needs.

Qualifications of the Social Worker:

• Master's (MSW) or Bachelor's (BSW) degree in Social Work
• Experience in a skilled nursing or long term care setting
• Comfort working with residents who have substance use and behavioral health needs
• Knowledge of psychosocial assessment, care planning and discharge planning
• Strong documentation and communication skills

Responsibilities of the Social Worker:

• Complete psychosocial assessments and the social services sections of the MDS
• Build and update care plans with the interdisciplinary team
• Coordinate discharge planning and community resources
• Support residents and families through admission, stay and transition
• Maintain documentation and regulatory compliance
