    ALS United Greater New York’s mission is to support and empower the ALS community through a collaborative approach to fostering bold research initiatives, advancing national and state advocacy, and providing comprehensive care to individuals and families affected by ALS. Our staff is professional and supportive of each other, as we work together to serve ALS patients and their families.

    Reporting to the Chief Care Services Officer, ALS United Greater New York is seeking an experienced licensed social worker to provide comprehensive support services and coordination of multi-disciplinary care services to help improve the quality of life of patients and their families living with ALS in Northern and Central New Jersey. ALS is a fatal and neurodegenerative disease that requires a compassionate and committed social worker who is comfortable with end of life care.

    In collaboration with other members of the NJ care team, this full-time, home-based position will conduct virtual and in-person home visits offering social work assessments, short-term supportive counseling, case management, and crisis intervention as well as weekly in person clinic roles in New Jersey.

    This position will also require the co-facilitation of support groups for patients and caregivers, as well as coordination of monthly educational programs all of which, at present, are being conducted virtually. In addition, the position requires periodic participation in chapter-wide events, some of which take place on weekends and evenings.

    QUALIFICATIONS:

    LSW with active NJ State license

    At least three years experience in similar role

    Experience in end of life care

    Strong communication and interpersonal skills

    Strong team player

    Bilingual a plus High degree of initiative, able to work independently

    Experience in group work

    Valid driver’s license and use of car
