Role Overview

The Resident Care Coordinator provides case management, care coordination, and behavioral support for adults with developmental and intellectual disabilities. This role focuses on resident transitions, retention, and holistic care planning, partnering closely with admissions, caregiving staff, and health & wellness teams.

Key Responsibilities

Coordinate person‑centered care plans (CQI)

Provide case management and behavioral/emotional support

Support caregivers through training and conflict resolution

Assist with admissions, transitions, and discharge planning

Collaborate on life‑skills classes, enrichment programs, and activities

Maintain communication with families and interdisciplinary teams

Qualifications

Bachelor’s degree in social work (BSW), Special Education, or related field

3-5 years’ experience supporting adults with developmental disabilities preferred

Background in case management, behavioral health, or mental health services

Strong leadership, counseling, and organizational skills

Commitment to ethical care and resident advocacy

Christian faith alignment preferred

Requirements

Safety‑sensitive position. Must meet state and organizational clearance requirements, including: fingerprint clearance, background check, drug screening, CPR/First Aid (or ability to obtain), and APS registry verification.
