Responsibilities: • Conduct in-home patient assessments to evaluate health needs and develop personalized care plans. • Administer medications, monitor vital signs, and provide skilled nursing care to patients in their homes. • Educate patients and their families on managing illnesses, injuries, and chronic conditions. • Collaborate with physicians and other healthcare providers to ensure comprehensive and coordinated patient care. • Document patient progress, treatment plans, and outcomes accurately and in a timely manner. Qualifications: • Active Registered Nurse license in the state of practice • Minimum of 1 year of experience in home health care • Proficient in electronic health record systems • Strong communication and interpersonal skills • Ability to work independently and manage a flexible schedule Compensation: $60 - $100 hourly
