    POSITION SUMMARY

    PIH Health Medication Management is a subspecialty providing focused chronic disease management care in collaboration with the primary care providers and specialists with Group and Affiliated providers. The Physician Assistant (PA) or Nurse Practitioner (NP) works independently and performs and documents a complete physical examination for acute and complex chronically ill patients. They administer preventive care and chronic illness surveillance. They assess, diagnose and manage health-illness needs specific to their managed chronic condition and managed by the Medication Management clinic.

    Compensation: Salary range $120k-$135k

    Required Skills

    SPECIFIC SKILLS NEEDED

    • Clinical expertise within chronic care management, specifically DM and anticoagulation
    • Ability to provide comprehensive patient assessments evaluate explain, teach, and problem solve using critical thinking
    • Demonstrates expertise in the 4M's: What Matters, Medication, Mentation, Mobility in caring for older adults
    • Advocates for "What Matters" for the patient, documenting, communicating and applying patient's wishes to guide their care
    • Excellent communication with both health care teams and patient, family and caregivers
    • Recognizes and proactively improve gaps in care and communication
    • Improve/meet goals of chronic disease management
    • Prevent and/or reduce avoidable ER visits and hospitalizations
    • Provide excellent communications across care settings, including outpatient and inpatient
    • Demonstrated leadership abilities
    • Independent performer

    Required Experience

    EDUCATION/EXPERIENCE/TRAINING

    • Current California Physician Assistant License
    • Graduate from an accredited Physician Assistant Program
    • Board Certification: Board certified by the National Certification Commission on Certification of Physician Assistants
    • Two to five (2-5) years previous experience in chronic care/primary care outpatient setting
    • Evidence of continuing education and self-development to support license requirements
