Children’s Mental Health Case Manager
Title: Children’s Mental Health Case Manager Department: Family Case Management Reports To: SCHRC Director of Children's Services/Children’s Services Supervisor
SUMMARY
DEFINITION: The case manager is responsible to assess client needs, plan for service implementation, coordinate services on behalf of clients, and monitor of services for consistency and efficiency. Case managers are expected to interact professionally with internal and external community providers, perform related work tasks as required or assigned and comply with Minnesota Rule 79 in deliver of Children’s Mental Health Case Management services.
Case manager may be assigned to perform job responsibilities in one or more than one of the counties under the jurisdiction of Minnesota Prairie County Alliance.
HOURS OF WORK: Primarily 8 am to 4:30 pm, (unpaid 30 minute lunch) Monday through Friday. This position allows flexible hours and may require evening hours, occasional weekend hours.
Education, Experience and Credentials
MINIMUM QUALIFICATIONS: Bachelor’s degree in one of the behavioral sciences or related fields, including but not limited to social work, psychology or nursing from an accredited college or university. *If the applicant has a degree in social work, they are required to have a current/active license prior to be considered for employment
PREFERRED QUALIFICATIONS: Bachelor’s degree as above, four (4) years experience in human services field, two (2) of those years as a children’s mental health case manager.
Compensation Details:
Education and experience may be considered when job offers are extended. The minimum pay rate is $ 22.68/hr
