Job Description: POSITION SUMMARY: The basic function of the Migrant Outreach Coordinator is to assist with developing, planning and coordinating the function of the department and to oversee the duties of the Outreach Case Managers and Drivers at their assigned sites. The Coordinator will focus on marketing for outreach events, cultural competency workshops and health education materials. DUTIES AND RESPONSIBILITIES: Monitor and evaluate outreach activities ensuring the proper operation of the outreach program and SJFMC policies Follow program procedures and activities as outlined in the Migrant Outreach Department Procedure Manual, Migrant Outreach Plan, Transport Policy Manual and carry out all grant related duties Collect and review weekly reports from Outreach Case Managers on a weekly basis Submit to Migrant Outreach Manager on a monthly basis program supply needs and patient satisfaction surveys (June to September) Document and inform Manager or other Administrative Staff about any incidents, accidents or problems pertaining to the Migrant Outreach Program Provide verbal and written feedback, including monthly reports, to manager regarding progress of the Migrant Outreach and program, including any recommendations for improving or changing any aspects of the programs Assist in the development of communication links between Outreach Case Managers, health personnel and service organizations within SJFMC and with the community as a whole Assist the Department Manager in recruiting, hiring, training, evaluating and terminating staff, development of reports and grants, and the provision of in-services and continuing education to outreach staff Assist with program operation as needed by performing outreach at migrant camps, community events and other community venues; perform individual and group health education sessions, serve as a liaison and/or interpreter between farm workers, community organizations and the health center staff; provide transportation to SJFMC patients, perform health screenings such as blood pressure, blood sugar, cholesterol, HIV testing, and other duties in order to ensure the smooth operation of the Migrant Health and Outreach program and assist SJFMC patients Assist in coordinating migrant patient care for visits, ordered tests and other healthcare needs Assist with migrant patients enrolled in case management Maintain electronic medical records (EMR) and other company information and ensure compliance with all State and Federal Guidelines Facilitate effective communication between migrant patients and providers Build and maintain relationships with both internal and external healthcare contacts and organizations in an effort to improve available patient resources Plan and promote outreach events at multiple sites, including farmworker appreciation events and farmworker health fairs Develop educational materials for health education workshops and implement health education workshops during the off-season Develop and execute cultural competency programs as needed, create tools and materials to strengthen cultural competency organization-wide, conduct cultural competency workshops for staff, new hires and community stakeholders Other duties as assigned or as needed to achieve departmental or organizational objective #ZR Experience and Skills: POSITION REQUIREMENTS: Excellent verbal and written communications skills with a focus on customer service Bilingual in Spanish and/or Creole Strong computer skills; knowledge of standard office equipment and technology in a Windows-based environment Valid drivers license and the ability to operate a motor vehicle Use of a registered and insured vehicle for local travel EDUCATION AND WORK EXPERIENCE: Bachelors degree in a relevant field At least 2 years of experience in community outreach or health education Case management experience preferred
