Works closely with medical providers, primary care teams and other agencies to improve patient care and outcomes. Effectively educates and links patients and providers to available community-based resources.
Possesses a current license to practice as a Licensed Practical Nurse or Certified Medical Assistant
Handles ethical challenges facing the individuals and communities they serve.
Protects confidentiality and privacy rights of individuals in the context of employer and legal reporting requirements.
Is aware of professional boundaries when balancing care for individuals and care for self.
Facilitates appointments for patients including arranging, coordinating, and facilitating transportation resources.
Ability to communicate both orally and in writing
Follows up on appointments or instructions from the healthcare team by phone communication
Must possess successful problem-solving and decision-making skills, as well as being able to multi-task.
Promotes wellness by providing culturally appropriate health information and education to patients.
Advances patients’ health literacy related to their conditions.
Encourages and supports patients to make steps toward promoting their health and self-managing their diseases and/or conditions.
Assists with medication management and adherence through the direction of the primary healthcare team.
Assists in identifying and obtaining clinical, social, and /or community services and resources for patients
Assists with post-hospitalization or emergency department visit follow-up by attempting to track down participants Primary Care Health Home staff have been unable to reach.
Adheres to policies/safety practices established by the Health Center
