    TGH Special Care Unit – Tampa General Hospital Special Care Unit is an elite team of acute care professionals who treat catastrophically ill, medically complex patients requiring extended acute care.     This position works with Partner Hospital/Insurer/SCU Leadership to identify and approve potential patients (in-house) who would benefit from services provided by the SCU. This position provides comprehensive assessment, planning, implementation and overall evaluation & coordination of individual patient needs from the preadmission process through discharge. This position must fully understand the admission criteria of the SCU and be capable of educating others on such criteria. This position must be able to effectively communication with hospital personnel, SCU leadership, Physicians and the interdisciplinary teams to develop a plan of care for each assigned patient from admission through discharge and able to effectively communicate this plan of care to the patient’s family. This role is key to ensuring that the patient is progressing towards the desired outcome by continuously monitoring patient results through daily assessments and/or evaluations.

    Job Qualifications:

    Education – BSN required

    Experience – 1 Year prior experience working in Case Management in a hospital setting.

    Licensure – Active State of Florida RN license

    Certifications – BLS and ACLS required
