    Undivided is seeking an Independent Facilitator for an exciting new company that offers the opportunity to directly impact families raising children with disabilities.

    Undivided provides care planning services, including system navigation, to parents of children with disabilities across California, with plans to expand.

    Under the direction of the Director of Navigator Services, the Independent Facilitator will work in collaboration with other Undivided teams to support parents with accessing resources and services through diverse and qualified networks of disability service providers, and to connect families within our unique, private online Community. The Independent Facilitator will guide families through the California Self-Determination Program enrollment. 

    Undivided will provide training and certificates required by the California Department of Developmental Services. 

    Primary responsibilities:

    • Conduct person centered interviews
    • Facilitate information capture to populate person centered plans
    • Create a person centered plan report to describe the child's needs, loves and required support elements
    • Identify unmet needs or changes in circumstances
    • Provide guidance to families through the budgeting process
    • Help families find providers and programs that meet their family needs
    • Collaborate with multiple departments to incorporate client needs and priorities into programming
    • Promote and amplify the Special X voice, mission, and culture of inclusivity

    Required education and experience: 

    • Master's-level social worker or equivalent education and experience is preferred. Recent graduates with special education, disability, and/or care management services are invited to apply.

    OR

    • Bachelor's degree in social work, psychology, family studies, human services, or related field; must also have a minimum of two (2) years of experience providing services to families of children with disabilities or directly to children with disabilities. Must have demonstrated experience in working successfully with special education, disability, and/or care management services.

    • Excellent verbal, written, and interpersonal skills.
    • Ability to handle sensitive information in a confidential manner.
    • Action-oriented with strong problem-solving skills.
    • Excellent organization skills and ability to multitask and juggle multiple priorities. Outstanding ability to follow through with tasks.
    • Ability to work cooperatively and effectively as part of an interdisciplinary team and independently assume responsibility.
    • Strong initiative and enthusiasm and willingness to pitch in whenever needed.
    • Ability to communicate well at all levels of the organization including working with leadership and high-level representatives of partner organizations.
    • Ability to work within a frequently changing project scope while maintaining overall direction and structured priorities.
    • Ability to show warmth and appreciation of human growth at all levels, and understand the process of gaining trust and credibility with individuals and families.
    • Ability to relate well to all people of the community regardless of color, national origin, religion, sex, pregnancy, age, marital status, sexual orientation, disability, or socio-economic level.
    • Computer literate with basic knowledge of Microsoft Word, Excel, data tracking programs, Salesforce, Slack, Google Suite, Vimeo, Zoom, etc. (preferred).
    • Knowledge of Special Education and services offered, care management principles, planning, resource development, benefits entitlements (SSI, General Relief, Regional Center, IHSS), motivational interviewing, strengths-based approach (preferred).
    • Must understand, uphold, and continue working toward accomplishing the mission, strategic goals, and performance measures of the organization.

    Preferred skills and knowledge: 

    • Bilingual (Spanish)
