Maintain accurate documentation and ensure continuity of care through direct 1-1 sessions and consistent patient follow-up.

Collaborate with multidisciplinary team members to ensure comprehensive patient care and optimal outcomes.

Qualifications:

Valid License for Physical Therapy in New Jersey.

Strong communication, interpersonal, and organizational skills.
