    Speech-Language Pathologist (SLP-CCC) - Skilled Nursing Facility | Hornell, NY

    Job Responsibilities

    • Conduct comprehensive speech-language evaluations and assessments for skilled nursing facility residents
    • Develop and implement individualized treatment plans for speech, language, cognitive-linguistic, and dysphagia therapy
    • Provide evidence-based therapeutic interventions to improve communication and swallowing function
    • Coordinate audiometric screenings and facilitate appropriate referrals
    • Collaborate with interdisciplinary team members including physicians, nurses, occupational therapists, and physical therapists
    • Communicate effectively with residents, families, and caregivers regarding treatment goals and progress
    • Maintain accurate and timely clinical documentation, progress notes, and Medicare compliance records
    • Participate actively in care planning meetings and interdisciplinary team conferences
    • Provide staff in-service education on speech-language pathology topics and swallowing safety
    • Contribute to quality improvement initiatives and program development
    • Ensure adherence to facility safety standards and infection control protocols
    • Deliver compassionate, dignified care that honors each resident's individual needs

    Job Requirements

    • Master's Degree (MA or MS) in Speech-Language Pathology from an accredited, ASHA-approved program
    • Active New York State Speech-Language Pathologist license in good standing
    • Certificate of Clinical Competence (CCC-SLP) from the American Speech-Language-Hearing Association (ASHA)
    • Strong clinical assessment and documentation skills
    • Excellent verbal and written communication abilities
    • Demonstrated professional maturity and ethical practice standards
    • Team-oriented approach with strong interpersonal skills
    • Commitment to ongoing professional development and evidence-based practice

    • Experience in skilled nursing or geriatric settings preferred but not required
