    Speech Therapist (Home Health) Position Type: PRN Location: Patient Homes Position Summary: The Speech Therapist (Speech-Language Pathologist) provides skilled assessment, evaluation, and treatment of patients with speech, language, voice, communication, cognitive, and swallowing disorders in the home setting. The therapist develops individualized care plans and collaborates with the interdisciplinary home health team to promote the patient’s functional independence and quality of life. Work Environment: This position involves providing care in patients’ private residences. The Speech Therapist must be adaptable to a variety of home environments and work independently while maintaining professional standards. Responsibilities: Essential Duties: • Evaluate and assess patient speech, language, communication, and swallowing function. • Develop and implement individualized speech therapy treatment plans based on patient goals and physician orders. • Instruct and counsel patients, families, and caregivers in communication techniques and swallowing strategies. • Collaborate with physicians, nurses, occupational and physical therapists, and other team members to ensure coordinated care. • Maintain accurate and timely clinical documentation in accordance with agency and regulatory standards. • Reassess patient progress regularly and modify treatment plans as appropriate. • Educate patients and families on home programs to maximize carryover and promote independence. • Participate in case conferences, staff meetings, and continuing education as required. • Adhere to all agency policies, procedures, and infection control guidelines. Qualifications: • Master’s Degree in Speech-Language Pathology from an accredited program. • Current State License as a Speech-Language Pathologist (SLP). • Certificate of Clinical Competence (CCC-SLP) from ASHA preferred. • Minimum of one year of experience , preferably in a home health or rehabilitation setting. • Strong interpersonal, communication, and organizational skills. • Reliable transportation and a valid driver’s license. Physical Requirements: • Ability to travel to patient homes. • Must be able to stand, walk, and lift light equipment as needed. Compensation: $42 - $55 hourly
