Speech Therapist (Home Health) Position Type: PRN Location: Patient Homes Position Summary: The Speech Therapist (Speech-Language Pathologist) provides skilled assessment, evaluation, and treatment of patients with speech, language, voice, communication, cognitive, and swallowing disorders in the home setting. The therapist develops individualized care plans and collaborates with the interdisciplinary home health team to promote the patient’s functional independence and quality of life. Work Environment: This position involves providing care in patients’ private residences. The Speech Therapist must be adaptable to a variety of home environments and work independently while maintaining professional standards. Responsibilities: • Conduct comprehensive assessments of patients' speech, language, and communication needs in their home environment. • Develop and implement personalized treatment plans that address each patient's unique challenges and goals. • Collaborate closely with the interdisciplinary home health team to ensure cohesive and effective patient care. • Provide education and support to patients and their families, empowering them to actively participate in the therapy process. • Monitor and document patient progress, adjusting treatment plans as necessary to achieve optimal outcomes. • Utilize evidence-based practices to deliver high-quality speech therapy services that enhance patients' quality of life. • Maintain clear and timely communication with healthcare providers, ensuring seamless coordination of care across disciplines. Qualifications: • Experience in evaluating and assessing speech, language, communication, and swallowing functions. • Ability to develop and implement individualized treatment plans tailored to patient goals and physician directives. • Proven track record of instructing and counseling patients, families, and caregivers effectively. • Strong collaboration skills with interdisciplinary teams, including physicians, nurses, and therapists. • Proficiency in maintaining accurate and timely clinical documentation in line with agency standards. • Capability to reassess patient progress and modify treatment plans as needed. • Experience in educating patients and families on home programs to enhance independence and carryover of skills. Compensation: $42 - $55 hourly
