The majority of our patients are in their personal homes we also have the pleasure of working together with inpatient facilities and we provide the following benefits: • Mileage Reimbursement • Career Development • PTO Responsibilities: • Assess the condition of patients and track their progress diligently • Create and execute a comprehensive care plan for each individual • Work collaboratively with the Hospice Team within an interdisciplinary environment • Provide education and emotional support to patients and their families • Maintain open communication with fellow healthcare professionals • Ensure compliance with regulatory and clinical documentation standards • Demonstrate the ability to work autonomously and as part of a team Qualifications: • Possess a valid Registered Nurse (RN) license in the state of New Jersey • Hold a Bachelor's degree in Nursing (BSN) • Have a minimum of one year of experience working in a Hospice environment • Previous exposure to hospice care is considered advantageous • Must have at least one year of medical and surgical experience • Demonstrate exceptional communication and interpersonal skills Compensation: $89,000 - $94,000 yearly
