    Seeking a compassionate Hospice Case Manager RN dedicated to delivering end-of-life care. Responsibilities involve providing comfort, guidance, and respect to patients and families. Duties include overseeing holistic care, emphasizing teamwork, communication, and clinical skills. Completion of all sections is essential for the application process. Responsibilities: • Conduct thorough assessments of patients in hospice care, including monitoring vital signs and reviewing medical histories. • Develop personalized care plans tailored to each patient's unique needs, considering medical evidence, cultural factors, and ethical considerations. • Administer medications and treatments following prescribed orders to ensure patient safety and well-being. • Educate patients and their families on health conditions, treatment plans, and preventive measures. • Collaborate with physicians and healthcare professionals to coordinate a comprehensive approach to patient care. • Maintain accurate health records, progress notes, and care plans in compliance with regulatory standards. • Perform clinical procedures such as IV therapy, wound care, and catheterization as needed. • Provide emotional and psychological support to patients and families, especially during challenging times. • Adhere to infection control protocols to minimize the risk of disease transmission. • Stay updated on medical advancements and maintain necessary certifications. • Contribute to quality improvement efforts by participating in audits and safety evaluations. Qualifications: • Must hold a valid RN license issued by the state of New Jersey. • Current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) are required. • Previous experience working in hospice care is highly preferred. • Excellent communication, interpersonal, and teamwork abilities are essential. • Demonstrated dedication to delivering compassionate care is a must. • Strong attention to detail and the capacity to remain composed in stressful situations are necessary. Compensation: $40 - $45 hourly
