Responsibilities: • Conduct thorough assessments of patients in hospice care, including monitoring vital signs and reviewing medical histories. • Develop personalized care plans tailored to each patient's unique needs, considering medical evidence, cultural factors, and ethical considerations. • Administer medications and treatments following prescribed orders to ensure patient safety and well-being. • Educate patients and their families on health conditions, treatment plans, and preventive measures. • Collaborate with physicians and healthcare professionals to coordinate a comprehensive approach to patient care. • Maintain accurate health records, progress notes, and care plans in compliance with regulatory standards. • Perform clinical procedures such as IV therapy, wound care, and catheterization as needed. • Provide emotional and psychological support to patients and families, especially during challenging times. • Adhere to infection control protocols to minimize the risk of disease transmission. • Stay updated on medical advancements and maintain necessary certifications. • Contribute to quality improvement efforts by participating in audits and safety evaluations. Qualifications: • Must hold a valid RN license issued by the state of New Jersey. • Current certifications in BLS (Basic Life Support) and ACLS (Advanced Cardiac Life Support) are required. • Previous experience working in hospice care is highly preferred. • Excellent communication, interpersonal, and teamwork abilities are essential. • Demonstrated dedication to delivering compassionate care is a must. • Strong attention to detail and the capacity to remain composed in stressful situations are necessary.
