    *PLEASE NOTE: THIS IS A FIXED TERM EMPLOYMENT POSITION THAT IS SET TO LAST 10 WEEKS.

    JOB SUMMARY

    The Workforce Development Case Manager works as a team member with social service professional and paraprofessional staff ensuring the delivery of quality care in accordance to the rules and regulations established as well as adhering to the mission of St. Joseph Center.

    ESSENTIAL DUTIES & RESPONSIBILITIES

    The Workforce Development Case Manager’s key areas of responsibility include: 

    KEY AREAS OF RESPONSIBILITY:

    • Provide therapeutic interventions to develop life skills, including, but not limited to money management, budgeting, accessing community resources, maintaining residence, and other skills as needed
    • Provide ongoing case management services that will ensure successful vocational goals, increase quality of life, and empower self-sufficiency
    • Provide advocacy and linkage to any needed or supportive services or resources, such as mental health, life skills training, self-help, volunteer services, job placement, etc.
    • Implement case management plan goals emphasizing coordinated strategies for addressing behavioral and circumstantial issues that could impede successful engagement with vocational programming
    • Provide ongoing assessment of client progress in attaining goal plans
    • Utilize a trauma informed, harm reduction, client focused and strengths based model
    • Respond to urgent requests for assistance from clients
    • Develop close working relationships with all collaborating agencies and internal programs
    • Develop and maintain strong collaborative relationships with other service providers involved in the client’s care and development

    ESSENTIAL DUTIES:

    • Provide case management and supportive services to meet the needs of each individual
    • Build trust and rapport with individuals and support with client engagement issues
    • Link clients to physical health, mental health, and other supportive services.  Assist clients to enroll in supportive benefit programs
    • Report on individual progress and case conference with program staff
    • Participate in case conferencing meetings and other community meetings

    QUALIFICATIONS

    KNOWLEDGE, SKILLS & ABILITIES:

    • Must be highly motivated and a self-starter.  The ability to communicate with and relate to a diverse group of people including clients, community, and other staff.  Must have excellent organizational skills and the capability to work in a fast paced environment
    • Strong knowledge of services and resources.  Demonstrated knowledge and experience with Trauma Informed Care, Harm Reduction, and Motivational Interviewing
    • A bilingual background is a plus
    • The position requires an ability to work flexible hours including some early mornings and evenings

    EXPERIENCE:

    • Two years’ experience providing services to highly vulnerable populations.

     

    EDUCATION:

    • Bachelor’s degree from an accredited college or university in psychology, social work, human services or a related field.

    DIRECT REPORTS:

    • This position has no direct supervisory responsibilities.

     

    COMPUTER SKILLS:

    • Proficiency in MS Office Suite (Word, Excel, PowerPoint, Outlook)
    • Ability to use widely supported internet browsers.
    • Ability to accurately enter data into Los Angeles County and St. Joseph Center databases.

    CERTIFICATES, LICENSES AND REGISTRATIONS:

    • Valid California Class C Driver License
    • Valid automobile liability insurance.

    TRAVEL REQUIREMENTS:

    • Travel to client sessions, meetings at multiple St, Joseph Center sites, and at community partners’ locations as needed

    Status: Full-Time/Non-Exempt

    Salary: DOE
