Case Manager - Housing Division (Limited Appointment)
Job Description

JOB SUMMARY
Under the supervision of the Program Manager, the Rapid Rehousing Case Manager will provide filed-based services to homeless individuals who are enrolled in our Rapid Rehousing Program. This program offers short-term rental subsidies combined with linkages to employment and other services in order to end clients’ homelessness as well as create longer term self-sufficiency. Once clients have stable housing through the support of the Case Manager and Housing Specialist, the Case Manager will also be responsible for providing ongoing services that will assist clients to retain their housing and be good tenants and neighbors.
