    JOB TITLE: Mental Health Specialist – HERO AGENCY: St. Patrick Center DEPARTMENT: Permanent Supportive Housing REPORTS TO: HERO Manager SUMMARY OF POSITION: As part of the Intensive Case Management (ICM) multidisciplinary team, the Mental Health Specialist will provide mental health assessments and case management to program clients with severe and persistent mental illnesses and chemical dependency problems. This specialist will provide individual and group counseling/therapy, family support/therapy, and psychoeducation. This specialist will assist with treatment planning and coordinate clinical services to ensure best practice service delivery. In addition, the specialist will work with team members to coordinate case management services, treatment, and referrals to other agencies. RESPONSIBILITIES: • Conduct comprehensive, ongoing assessments of program participants mental illness, physical health, housing status and other relevant needs, where appropriate • Provide direct clinical services to clients on an individual and group basis in the office and in community settings, making appropriate changes in treatment plans to ensure the client’s ability to cope with internal and external stresses • Refer clients to appropriate community agencies, assisting clients when needed • Act as liaison and consult with community agencies and program clients to maintain coordination in the treatment process • Collaborate with other members of the ICM team during daily and weekly staff meetings • Facilitate treatment planning groups, participating in setting treatment goals and plans with clients with an emphasis on facilitating personal growth and development towards recovery and wellness • Provide on-call crisis intervention in coordination with other ICM members, covering nighttime hours and weekends • Conduct weekly or monthly follow ups on all program placements and document progress • Complete daily and monthly departmental paperwork • Maintain St Patrick Center Client Tracking System and client files, documenting client progress to maintain a permanent record of client activity • Assist team members with life skills classes • Responsible for a case load of 5-10 clients • Be able to respond to after hour emergencies and crisis intervention • Assist team member with budgeting and other supportive services • Research additional benefits available to enhance services • Responsible for the completion of departmental goals of mental health specialist as outlined • Responsible for maintaining records and statistics for the completion of annual reports • Ensure the care model by interacting with all other St Patrick Center programs • Must possess a Missouri Class E Driver License (MO residents only) • Other duties as assigned DIRECT REPORTS: • None KNOWLEDGE AND EXPERIENCE: • Master’s degree in related area from an accredited university or college; Licensure or Credential in Social Work or Counseling required. • Understanding of mental illness and substance abuse • Trauma Informed Care Trained • Experience with wrap-around services preferred • Experience in conducting home visits • Knowledge and understanding of area/community resources to provide appropriate referrals • Knowledge of federal, state, and agency HIPAA policies • Available to work a minimum of 37.5 hours a week • Reliable transportation is required SKILLS AND ABILITIES: • Good communication skills • Counseling and crisis intervention expertise • Self motivation • Respect for clients and staff • Sensitivity to cultural diversity • Able to work in a team atmosphere • Moderate computer skills RELATIONSHIPS REQUIRED: • Report to and work with HERO Manager • Attend and participate in all appropriate St. Patrick Center meetings • Works with other ICM staff • Develops relationship(s) with client family and natural supports • Works with other Permanent Supportive Housing staff • Works with the staff in other SPC programs • Networks with federal, state and local agencies • Embracing of St. Patrick’s mission and the dignity and worth of all persons
