    Behavioral Health Counselor -Shamrock ICM Job Title: Behavioral Health Counselor Reports to: Director of Behavioral Health Programs Date of Review: March 13, 2020 Summary of Position: This counselor will utilize wrap-around philosophy and techniques to assist clients with mental health & substance use disorder/recovery challenges who are transitioning from homelessness to self-sufficiency and stability. This counselor will meet with clients to provide individual and group therapy counseling as well as helping the client to identify family and natural supports in the community, collaborate to develop an Individual Service Strategy/Treatment Plan of Care, coordinate individualized services for affordable housing placement, coordinate the involvement of providers working in the home and community setting, and document all support related to activities and services. Duties and Responsibilities: Identify family, natural, and wrap-around supports  Conduct interviews with clients to develop a treatment plan  Provide on-going counseling/case management services for a caseload of clients  Contribute to the ideas and processes of the ICM team; collaborate with the ICM team  Assist with housing placement based on the desires of clients  Assess the client’s strengths, desires, and immediate/long term needs  Develop or contact family and natural supports which exist in the community  Receive input from ICM team regarding employment, housing, mentoring, substance use, and other immediate/long term needs  Assist in the daily running of treatment program (Shamrock Club) Document all referrals or services provided to client into CaseWorthy/ServicePoint systems  Complete departmental paperwork.  Update daily clients’ electronic files in CaseWorthy/ServicePoint to include any types of contact with clients, services, or any internal or external referrals made Provide Case Management  Adhere to the strategies and goals outlined in the ISS and Treatment Plan  Address client needs by providing necessary referrals  Coach clients to make solid, well-informed decisions Perform Outreach  Refer client to additional resources to address other service needs Qualifications: Interest in helping clients reach full potential, Ability to work well with a wide range of people, Good planning skills, Ability to handle confidential information, Ability to remain calm in stressful situations, Ability to process large volumes of paperwork Direct Reports/Supervisory Relationships: None Skills, Knowledge & Experience: College degree: counseling psychology, BSW or related field preferred; perform effective case management; knowledge of behavioral health issues and substance use disorder issues required. Knowledge of housing and other supportive service resources; ability to engage and work with community programs.
